Northeast Valley Health Corporation serves as the lead agency in managing one of the country’s largest Health Care for the Homeless grants for the Cooperative Health Care for the Homeless Network (CHCHN).  

The 14 health care agencies that comprise the CHCHN are located throughout Los Angeles County and provide comprehensive health care services to at least 41% of the County’s homeless population.  Over the past twenty-five years, the Network has worked with homeless healthcare partners to provide medical care, dental care, case management, behavioral health care services, alcohol/substance abuse treatment and outreach to those in need, regardless of their ability to pay for services.  

The Network also maintains a unique Quality Management Program specific to the homeless population and chronic disease management.

Services

The Cooperative Health Care for the Homeless Network (CHCHN) provides primary health care, outreach, substance abuse and mental health services, case management, entitlement, eligibility assistance, provision of and arrangements for emergency health services, and referrals for inpatient hospitalization.

The Network also maintains a unique Quality Management Program specific to the homeless population. Reports include individual and Network achievements as well as trending data with specific focus on chronic disease management.
