The mission of Homeless Health Services (HHS) is to provide comprehensive primary health care to people experiencing homelessness in the Fargo-Moorhead area.

Services

HHS is a program of Family Health Care Center in Fargo. The project maintains a primary care clinic in the downtown area at St. Mark’s Lutheran Church. Clinic hours are Monday through Friday, 8:30 am to 12:30 pm. Services include management of acute and chronic health problems, preventive care, and patient education. Patients receive case management services, including referrals for mental health and substance abuse services. Outreach is provided by staff at area shelters and other community sites serving people who are experiencing homelessness. 24 hour emergency on-call, Asthma treatment, Basic Needs, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Documenting Disability Services, Domestic Violence services, Electronic Medical Records, Family Planning, Financial counseling, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Housing placement, Immunizations, Interpretation services, Labs, Medication Prescribing, Medication Dispensing, Mental Health Counseling, Needle Exchange, OB/GYN, Outreach, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, Services in permanent supportive housing, Sexual Abuse Support Services, Shelter outreach, STD Testing, Street Outreach, Substance Abuse Treatment, TB screening/education, Tobacco cessation, Transportation services, Urgent Care, Well Child Check-ups, X-rays

Homeless Health Services has linkages with Family Health Care, Fargo Cass Public Health, Merit Care Medical Center, Southeast Human Service Center, Lakeland Mental Health, area shelters, and transitional housing sites.
