Peak Vista Community Health Centers
Peak Vista Community Health Centers provides comprehensive, integrated health care through its Health Center Downtown in Colorado Springs. Formerly known as the Homeless Health Center, this location transitioned between 2018 and 2022 to reflect an expanded, inclusive care model serving the broader community.
The Health Center Downtown offers preventive and primary medical care, behavioral health services, and dental care for patients of all ages. Care is delivered through a team-based, integrated model that emphasizes continuity, coordination, and whole-person care, regardless of a patient’s housing status or ability to pay.
Located intentionally in downtown Colorado Springs near key community partners, the Health Center Downtown improves access to care for individuals who may face barriers such as housing instability, limited transportation, or lack of insurance—while providing the same high standard of care to all patients.
Services
The Health Center Downtown provides integrated, outpatient health services designed to support prevention, early intervention, and ongoing management of health needs for individuals and families. Services include preventive and primary medical care for patients of all ages, chronic disease management, behavioral health care with integrated mental health support, dental care for children and adults, and care coordination with referrals to specialty and community-based services as needed.
All services are delivered on-site through a multidisciplinary care team using an integrated medical record system to support continuity, quality, and patient-centered care. The Health Center Downtown does not operate mobile clinics or street-based outreach programs, but works collaboratively with community partners to help patients access appropriate services.
Linkages
The Health Center Downtown maintains strong linkages with community organizations, social service agencies, and specialty care providers throughout the Pikes Peak Region to support comprehensive care beyond the clinic setting. As appropriate, patients may be referred to specialty medical and behavioral health providers, substance use treatment services through community partners, hospital and inpatient services, dental specialty care within the Peak Vista system or through external referral partners, and social support resources addressing needs such as housing stability, food access, transportation, and other social determinants of health.
Through coordinated referrals and partnerships, the Health Center Downtown supports continuity of care while remaining focused on delivering integrated outpatient medical, dental, and behavioral health services.
