Gardner Health Services, Inc. Healthcare for the Homeless Project (GFHN-HHP) provides medical, case management and substance abuse services to its clients. Primary health care services include routine physical assessments, treatment and management of chronic health problems, acute care, women’s health services, TB, STD, HIV testing. Men, women, and children receive culturally sensitive health care, social and medical case management, referrals for specialty care, health education, and substance abuse services at scheduled rotating locations throughout Santa Clara County via mobile medical unit. Services are also provided at four fixed sites, including the Cathedral Clinic. GFHN-HHP collaborates with the homeless service community to strengthen the quality and scope of services. Other services include translation assistance, client advocacy, referrals for basic needs, shelter, and housing assistance. Clients needing services beyond the scope of GFHN-HHP are referred to other community agencies.

Healthcare for the Homeless Project

Our medical team provides a variety of preventive and primary healthcare services to homeless individuals including substance use treatment and counseling, case management, and urgent care referrals. The program also offers eligibility and registration services through homeless shelter locations in the Santa Clara County. Our services are provided by two medical mobile units, at John XXIII Service Center, and at two intermittent clinics in partnership with City Team and the Bill Wilson Center

Linkages

GFHN-HHP is in partnership with the Basilica St. Joseph in San Jose. The Diocese has provided space for a clinic to serve the homeless, in conjunction with their social service model, serving the same population. In 2013 we celebrated 25 years of partnership with the Diocese, as the Homeless Project observed 25 years of healthcare services via this homeless healthcare grant. CityTeam Ministries and Bill Wilson Center house intermittent clinic sites that are utilized at least once per week.  The GFHN-HHP mobile medical unit links with and serves: Martha’s Kitchen, Family Supportive Housing, Salvation Army Adult Rehabilitation Center, InnVision Commercial St. Inn, InnVision Montgomery St. Inn, Salvation Army 4th Street, Mt. View Day Worker Center, Gilroy Compassion Center, Gilroy National Guard Armory, St. Joseph’s Family Center Gilroy, several migrant camps in Gilroy to include; Arturo Ochoa, Red Camp, No Name Uno and others. Other partners include the General Assistance office in San Jose
