Crusader Community Health provides primary care and supportive services to individuals who are homeless at each of our clinic sites. Crusader’s Healthcare for the Homeless service aims to increase access to healthcare to homeless individuals in our community by decreasing barriers to care, offering comprehensive case management, and individualizing care by recognizing each patient’s unique circumstances and needs.

Services

Crusader Community Health has multiple primary care clinics, including school-based health centers, and works with other community agencies to ensure access to healthcare throughout Winnebago and Boone Counties. Case managers work with patients to assess patient needs, create an individualized treatment plan, and follow patients over time to ensure best outcomes. As needed, case managers work with patients to address many of the following needs: coordinating needs for primary and specialty care, improving healthcare utilization, assistance with accessing medications; assistance with securing transportation for healthcare needs; linking patients with appropriate behavioral health resources, identifying and navigating resources to meet social determinant of health needs, including shelter, housing assistance, food, employment, legal aid resources, benefits and entitlements.
Case managers assist with increasing access to care through routine outreach and coordination with local agencies, including Rockford Rescue Mission, Carpenter’s Place, Jubilee Center, and Remedies Renewing Lives.

Linkages

Crusader Community Health coordinates with local homeless shelters, mental health agencies, substance use treatment facilities, food pantries, housing authorities, state and federal agencies, and many other human service agencies.
