Downriver Community Services, Inc.

Downriver Community Services Homeless Health Care has been providing primary and preventative healthcare services to the Macomb and St. Clair County areas since 1975. Our Homeless Healthcare program offers health care services to the community in both a facility-based and community-based setting. These services include: Primary Health Care, Lab & X-Ray Services, Transportation Assistance, Translation Services, Routine and Emergency Dental Care, Women’s Health Services, Behavioral Health Services, Maternal Infant Health Program, WIC Services, and Medication-Assisted Treatment through our Vivitrol Program. 

All of these services are administered by medical professionals comprised of Primary Care Physicians, Nurse Practitioners, Behavioral Health Therapists, a Clinical Social Worker, and Community Health Workers conducting outreach and enrollment services in the field.

Services

The Mobile Health Team, comprising a full-time nurse practitioner and two full-time RNs, conducts outreach and refers patients to Downriver Community Services, New Haven Medical Center, People’s Clinic for Better Health, and Neighbors Caring for Neighbors for primary care and substance abuse services. In addition, clinics have been established in MATTS shelter and McRest Shelter.

Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Family Planning, Flu Vaccinations, Gynecology, Health/Nutritional Education, Hepatitis C screening/treatment, HIV/AIDS screening/education, Immunizations, Labs, Mental Health Counseling, Mental Health Services for Youth, Mobile Van/Transportation, Multidisciplinary Health Teams, Obstetrics, On-the-job training, Outreach, Pain Management Services, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Well Child Check-ups, X-ray

Linkages include shelters and soup kitchens, Downriver Community Behavioral Health, Downriver Community Behavioral Health, Downriver Community Substance Abuse Services, Community Mental Health, Clearview Residential Substance Abuse Treatment Center, and Harbor Children’s Shelter.
