Community Health of South Florida, Inc
Community Health of South Florida, Inc (CHI) provides a full spectrum of comprehensive primary and behavioral health care services at seven (7) modern facilities and 27 school-based health centers. CHI also operates two Urgent Care Centers (UCC) at the Doris Ison South Dade Health Center and Martin Luther King Center (MLK). Services are provided at Doris Ison seven days weekly from 2:00 pm to 10:00 pm, and at MLK six days (Monday-Saturday) weekly from 2:00 pm to 10:00 pm.
CHI is Joint Commission accredited for primary and behavioral health care and laboratory services. CHI provides family medicine, OB/GYN and deliveries of the pregnant patients, pediatrics, internal medicine, dental, adult and children mental health, substance abuse counseling, and HIV/AIDS testing and treatment. Enabling services include case management, navigation, health education, Medicaid eligibility and application, transportation and translation. CHI receives funding from the Bureau of Primary Health Care (BPHC) to provide health care services to special populations to include Homeless and Migrant Seasonal Farm Workers. CHI’s outreach program is a critical component to create access for the target population and schedule medical appointments.
CHI provides comprehensive primary and behavioral health care services, including internal medicine, family practice, pediatric, OB/GYN, dental, mental health/substance abuse, and HIV/AIDS testing, treatment and counseling. The project’s potential clients constitute 3,000 migrant and seasonal farm workers and homeless individuals. In addition to addressing health issues, CHI’s services include school absenteeism, adolescent high risk behavior, pesticide exposure, and communicable disease. Outreach is a critical component for identifying new homeless users.
