Senior Medical Director of Primary Care (Portland, OR)

The Senior Medical Director of Primary Care holds the accountability in a partnership dyad with the Senior Director of Primary Care Operations and Clinics for the delivery of all primary care services provided at Central City Concern.  The Senior Medical Director of Primary Care leads and mentors the organization’s primary care clinical staff, and sets strategic direction in partnership relating to all aspects of primary care services with the Senior Director of Primary Care Operations and Clinics, the Chief Medical Officer, the Executive Health Services Operations Leader, the Office of Integration and Innovation, and the Department of Quality. 

The Senior Medical Director of Primary Care also establishes clinical culture and promotes the professional, clinical, and ethical values and standards to which all Primary Care Clinical staff are expected to adhere.  The Senior Medical Director of Primary Care promotes the quality and safety of care, establishes and maintains the organization’s standards of clinical care, and has the responsibility for the clinical culture of service excellence, patient safety and continuous improvement.

Scope of Job:

Clinical Activity and Leadership:

  • Provides direct patient care 0.6 FTE in a team setting at Old Town Clinic and the Blackburn Center. 
  • Holds accountability for the cultivation and management of a workforce culture that drives for clinical performance of the primary care division to clinical service delivery targets and other quality and performance goals or targets identified by Executive Health Services Leaders, the Office of Integration and Innovation and the Department of Quality.
  • Serves as clinical leader for primary care Licensed Medical Providers (LMPs) and other primary care clinical staff at Old Town Clinic and the Blackburn Center, including availability for brief clinical consultation, provision of clinical teaching, clinician mentoring, and modeling of exemplary clinical practice.
  • Serves as the clinical leader of CCC primary care services and is engaged in the discernment, development and strategic planning required to successfully meet payer-based contractual priorities and performance goals.  

Management and Supervision of CCC’s Licensed Medical Providers and Associate Medical Directors of Primary Care:

  • Responsible for the recruitment, evaluation and retention of Medical Director/Associate Medical Directors of Primary Care who supervise programs at OTC, the Blackburn Center, shelter-based clinics, and the Recuperative Care Program.  Provides support and leadership for the Associate Medical Directors in this process for programs they supervise as needed.
  • In collaboration with the Associate Medical Directors of Primary Care, manages and supervises primary care providers in all primary care professional and clinical domains, excluding providers on the behavioral health team.
  • Engages in regular meetings with Associate Medical Directors and LMPs he/she directly supervises to observe, mentor and coach these front-line teams in a manner that creates a culture of accountability, fosters continual improvement and staff wellness.
  • In collaboration with the Associate Medical Directors of Primary Care, provides medical direction and clinical supervision to CCC’s primary care providers in accordance with state regulations and standards of practice.  Ensures all providers receive appropriate staff development, in-service training and performance review.
  • In collaboration with the Associate Medical Directors of Primary Care, leads the development and teaching of standard work related to medical practice for LMPs based on best practice, continuous learning and improvement, and compliance with rules and standards of practice.
  • In collaboration with the Associate Medical Directors of Primary Care, assures, through an interdisciplinary process, the appropriate credentialing and privileging of all LMPs.
  • In dyadic partnership with the Senior Director of Primary Care Operations and Clinics, leads the implementation of consistent care team models of care across the primary care continuum, including LMP roles for standardized best operational practices.

Assurance of Superior Quality of Medical Care

  • Responsible for the development and the representation and promotion of all medical practice guidelines, policies and protocols for Primary Care at CCC through the Clinical Standards Committee.  
  • Identifies, selects and oversees implementation of clinical evidence-based practices and “best practices” across CCC Primary Care programs.
  • Supports the development of new clinical programming pursuant to clinical excellence in the provision of primary care services within CCC.
  • In collaboration with CCC’s Clinical Safety Manager and other primary care leadership, responsible for clinical safety monitoring, ongoing clinical risk assessment, and promotion of just culture within CCC’s Primary Care programs.
  • In collaboration with other primary care leadership, responsible for monitoring, and improving/maintaining financial performance metrics within CCC’s Primary Care programs.
  • In collaboration with other primary care leadership, responsible for monitoring and promoting improvement or maintenance of local/federal clinical performance metrics and certifications (ie HRSA, NCQA, PCPCH, CCO) within CCC’s Primary Care programs.
  • Provides clinical leadership as a member of the CCC Controlled Substance Review Committee.
  • Adheres to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.  Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.  This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator.


  • License to practice medicine as a medical doctor or osteopathic physician, in Oregon.
  • Required: Minimum Four years’ experience providing care or consultation in integrated primary care/behavioral health setting.
  • Required: Minimum Four years’ experience in established leadership capacity within a healthcare setting
  • Preferred: Minimum Seven years’ experience in providing medical care as a medical provider for individuals in a Primary Care, Public Health.
  • Preferred: Clinical experience providing services and/or leadership within an FQHC and/or an HCH homeless clinic setting.
  • Preferred: Experience providing Medication Assisted Treatment (MAT) in a primary care setting with Buprenorphine/Naloxone or Buprenorphine.
  • Current BLS certification or must obtain before start date if not current.
  • Must have full prescriptive powers in Oregon with a DEA number at the time of start and a DATA X-Waiver within 45 days of start.
  • Must have admitting privileges at a local hospital or obtain within 90 days of start date.
  • Must meet CCC privileging requirements as required by FTCA.
  • If a recovering chemically dependent person, a minimum of two years sobriety is required.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Able to climb stairs several times a day.
  • Must pass a pre-employment drug screen, TB test, and background check.
  • Must adhere to agency’s non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations, and treat each individual with respect and dignity.
