Job description

West County Health Centers Inc. is a Federally Qualified Health Center (FQHC) and a national leader in transforming community health centers to patient-focused centers of care. WCHC has a consistent record and reputation of providing the highest standard of care within a community health care setting. WCHC provides access to comprehensive health care services at all our 8 community health center locations throughout West Sonoma County.

We have an immediate opening for a Behavioral Health Specialist as an integral part of the Healthcare for the Homeless Team. As an integral part of the Primary Care Team, you will:

  • Works with the HCH Primary Care team and other staff to provide assessment, intervention, and brief and ongoing, solution-focused interventions, including psychotherapy to patients with medical and behavioral health problems within the primary care and HCH setting.
  • Provides crisis assessment and intervention when needed.
  • Provides outreach and engagement activities as needed.
  • Participates in care coordination, patient education and information and referral.
  • Engages collaboratively with community-based and government agencies to assure continuity and coordination of care for your patients.
  • Provides facilitation of groups for individuals with similar diagnoses and symptoms.
  • Documents care in the medical record in compliance with ethical and legal standards.
  • Participates in data collection and management to track clinical activities as needed.
  • Participates in consultation, peer review, and team activities and meetings.

Experience & Qualifications

  • Possession of clinical licensure in good standing for the State of California for Social Work (LCSW) or Psychology (PhD/PsyD).
  • Experience working with diverse populations providing crisis, brief and longer-term individual, family and couple’s counseling and intervention.
  • Experience working with persons experiencing homelessness or who are at risk of becoming homeless.
  • Experience in health care settings preferred.
  • Training/experience with Chemical Dependency multiply diagnosed individuals.
  • Basic office and equipment safety knowledge and practice.

Physical Requirements

  • Must be able to move freely about the office and carry light loads (up to 25 pounds)
  • Prolonged standing and occasional sitting are required.
