Camillus Health Concern, Inc. (CHC) provides a comprehensive and multidisciplinary range of primary, preventive and supplemental health services for all patients of all ages. Comprehensive services are provided at CHC’s main site in downtown Miami and primary care services are provided at four service provider sites for homeless individuals: Better Way of Miami (substance abuse treatment center), Mother Theresa’s Sisters of Charity (social services and shelter facility), Salvation Army (social services and shelter facility) and Camillus House Shelter Site (social services and shelter facility). Services include primary adult and pediatric medical care, women’s preventive services, behavioral health, oral health, podiatry, dermatology, respite care, case management, benefits eligibility services, HIV/AIDS screening and treatment, and transportation. Outreach services are also provided at street and shelter sites throughout Miami-Dade County. CHC ensures t hat patients have access to comprehensive services by working with its sister agency, Camillus House, which offers meals, emergency shelter, transitional housing, and substance abuse treatment, along with other community-based agencies. Together Camillus Health Concern and Camillus House offer comprehensive health and social services for persons who are homeless.

Services

CHC has five fixed sites and a number of street sites. Services are provided at its main site in downtown Miami and at four service providers for homeless individuals: Better Way of Miami (substance abuse treatment center), Mother Theresa’s Sisters of Charity (social services and shelter facility), Salvation Army (social services and shelter facility), and Camillus House Shelter Site (social services and shelter facility). CHC has two full-time outreach teams – one focusing on medical street outreach and the other on HIV prevention education. Outreach services are provided at numerous street and shelter sites throughout Miami-Dade County. CHC provides adult and pediatric primary health care, respite care, women’s preventive services, oral health, behavioral health, podiatry, specialty services, case management, benefits eligibility services, and transportation. CHC ensures that patients have access to comprehensive services by working with Camillus House, which offers meals, emergency shelter, transitional housing, and substance abuse treatment, along with community-based agencies. Linkages

CHC maintains extensive collaborations and linkages with a variety of partners in the community, including Jackson Memorial Hospital, Florida Department of Health, health professions training programs with the University of Miami School of Medicine, and various social service providers serving individuals who are homeless.
