Mission Hills
Harbor Recuperative Care (HRC) is located in Mission Hills, CA. The HRC offers 30 beds of shelter for injured or ill homeless individuals nearing release from Hospitals. The HRC provides Intensive Case Management and Housing location services for its clients.

San Francisco
The mission of the Medical Respite and Sobering Center is to provide recuperative care, temporary shelter, and coordination of services for medically and psychiatrically complex, homeless adults in San Francisco. The San Francisco Medical Respite Program provides recuperative services for hospitalized homeless persons who are too medically frail to return to the streets but who do not require further hospitalization or skilled nursing facility care.

San Jose
We are a 20 bed facility located within a large homeless shelter with an on-site clinic. Our clients stay in double occupancy rooms with meals provided by the shelter. Our staff include social workers, nurses, psychologists, a pharmacist, community health worker, and part-time medical director. All clinical services are provided within the clinic on-site.

Eureka
St. Joseph Health Humboldt’s Medical Respite Program manages 17 respite beds in three locations. The program is facilitated by St. Joseph’s Care Transitions program to provide assessment, care coordination, case management and navigation assistance to homeless patients after discharge. The Care Transitions staff screens potential patients to assess for eligibility and to determine which location is the best fit for the client after discharge.

Novato
Most people leaving the hospital after surgery or illness don’t go straight back to work or school; usually they rest and recuperate at home to fully heal and regain strength. For people without homes to call their own, there is no stable place to transition to wellness. In a unique collaboration with Marin General Hospital, Kaiser Permanente, Novato Community Hospital, Ritter Center, and the County of Marin, Homeward Bound of Marin has created three medical respite rooms with two hospital beds in each room so homeless adults can recover their health in a safe environment.
