LTHC has partnered with community healthcare providers to create a Medical Respite Program within a 24/7 Homeless Services Engagement Center. The Medical Respite program is designed to care for men and women after they have had a hospital stay for an illness, injury or surgery in which they still have recovery needs and are still not well enough to resume life in a shelter or on the streets. The four medical respite beds provide a place where men and women can convalesce in a compassionate, supportive, and “home like” environment that is designed to care for their needs in a holistic manner. The Medical Respite Program is overseen by a Community Health Worker and provides medical care from community healthcare partners and nursing students from local schools.

In order to be eligible to utilize the Medical Respite Program, a person must meet the federal definition of homelessness,
– Be able to complete Activities of Daily Living, such as: dressing, bathing and eating without assistance.
– Have an acute health condition, such as recent surgery that is approved by the Community Health Worker as being in need of services.
