Ascending to Health Respite Care (ATHRC) is the only organization in the Pikes Peak Region that gives homeless individuals a safe place to recuperate after being discharged from local hospitals. ATHRC offers three programs to help prevent readmissions to the hospital, improve patient recoveries and health outcomes.

Medical Respite – Patients access medical care and support services which include connections to primary care, case management, benefits enrollment, nutrition, transportation, behavioral health providers and long term supportive housing options.

General Clinic Services/Behavioral Health Focus -The Boulder Clinic provides patient-centered care focused on mental health and substance abuse conditions using trauma-informed care and motivational interviewing as a cornerstone in developing treatment modalities. The service is available to homeless individuals in the community.

Permanent Supportive Housing –
A Step Up provides patients leaving respite care with advancing morbidities (cancer, dementia, end-stage-COPD) stable housing and multi-tiered case management to offset multiple hospital readmissions, enabling them to approach end-of-life status with dignity.

Admission Criteria:

*Age 18 or over
*Referred from local hospitals or from local homeless service providers for those released within 72 hours of hospital discharge.
*Does not require more than minimal assistance with ADL’s
*Willing to come to the Medical Respite Center, abide by facility rules, and participate in own care
