The North Broward Hospital District (d.b.a. Broward Health) Health Care for the Homeless (HCH) Program provides respite care services through a contractual agreement with an Assisted Living Facility (ALF) that provides respite and recuperative care to our clients. Clients must HRSA definition of homelessness, in need of respite or recuperative care, have no income or insurance, and be compliant with facility rules. If the patient requires nursing care, the hospital will arrange for home health nursing to follow patient at the facility for medical care until medically cleared. While patients are in the ALF, follow-up medical care services are provided at the HCH Bernard P. Alicki Health Center as needed. During intake, new patients are registered in the primary care system so that they can follow up at the health center after discharge for ongoing medical care. The Respite Care Coordinator receives referrals, assesses patients to determine if appropriate for the respite care program, monitors patient’s stay and level of need, and upon discharge assist with appropriate housing placement.

Admission Criteria:

18 years of age or older; short-term medical needs only; no long-term medical care needs, substance abuse, or mental health diagnoses or conditions are accepted.
