The Health Recovery Homes (HRH’S) opened in July 2000 to service homeless men needing a place to recover after in inpatient hospital stay. Providing a vital link in the continuum of care, the HRH serves the needs of the inpatient well enough to go home from the hospital, but having no home to go to.

In January 2016, we opened our first women’s HRH. Our respite homes are based on a holistic community home health model, providing a safe environment to recover health. Stabilization of presenting medical conditions and sustainable access to on going health care is the focus. Residents are connected to supports needed to live a healthy, productive life after their short term recovery stay at our men and women’s HRH’s.

Admission Criteria:

Patients must be deemed homeless at time of discharge from the hospital.
Patients primary diagnosis must be related to a medical condition
Patients must be able to perform basic daily living skills
Patients must be continent of bowel and bladder
Patients must be able to live and function independently in a shared living environment.
Patients must be able to ambulate independent with staff assistance
