Yakima Neighborhood Health Services provides low-barrier recuperative housing, medical oversight, and support services for patients needing care in lieu of hospitalization and for patients needing additional recuperation after hospital discharge. Our units are stand-alone apartments, overseen by RNs, Behavioral Health Specialists, Case Managers, and Housing Specialists, and a 2 blocks away from our HCH primary care clinic (medical/dental/behavioral health).

Units are individual apartments. No staff onsite after hours, however telephone access to providers and case managers (MDs/ARNPs) is available 24hrs/7 days

Admission Criteria:

Patients must have an acute medical problem that would benefit from short-term respite, be independent in ADL’s including medication administration, independent in mobility, continent, medically stable, behaviorally appropriate to be left alone, no IV lines, and does not need Skilled Nursing Facility placement.
