Description:

Arches to Wellness is a medical respite program supported through WellSpan Health, providing 24 hour care and services for 8-10 individuals with short term medical needs that are homeless or have housing instability. Individuals accepted into the program must be able to independently manage most of their activities of daily living and willing to comply with the structure of the program. The short term program is intended to last for 45 days or less, and accepts patients transitioning from any acute care setting in WellSpan Health’s multiple county service area. The program operates in two locations in York County. Both locations are housed within a Personal Care Home setting that traditionally provides long term care and services to care individuals with low income. The units are fully accessible and located near public services and transportation.

If the residents have a prior or ongoing relationship with a primary care physician, the program supports continued medical care with that provider. Residents without a primary care physician are supported by the clinical care team. All residents are connected with supports and services aimed to stabilize health conditions and help meet resident goals for housing, income, and community integration.

Alert and Oriented, able to manage most activities of daily living independently, able to manage or learn to manage medications, willingness to participate in program and comply with facility criteria and rules
