Rebranded as Avenue 360 Health & Wellness in 2016, Houston Area Community Services, Inc. (HACS) is a Federally Qualified Health Center founded in 1997 in the City of Houston, Harris County, Texas. HACS’ mission is to provide high quality and caring service to promote healthy people and communities.

With 3 health centers in Houston, HACS is accredited by the Joint Commission for Ambulatory Care and Behavioral Health, and recognized by the National Committee for Quality Assurance and Joint Commission as a Primary Care Medical Home. The state licenses HACS’ Class A Pharmacy, X-ray services, and its Intensive Outpatient Treatment (IOT) program for adult Substance Abuse Services.

HACS proposes to continue providing services in the current service area to the targeted population, which includes uninsured and underinsured individuals below 200% of the federal poverty level (829,495 individuals), public housing residents, and individuals experiencing homelessness.
