Community of Practice Description

End-of-life carries different meanings for different individuals, and each person has their own preferences based on their values and beliefs. The limited availability of adequate end-of-life care facilities is a growing problem for the general population, and there is even more concern about the lack of resources available within the homeless services continuum. End-of-life care presents a unique opportunity for collaboration between health centers and medical respite care programs.
The End-of-Life Care Community of Practice provides an opportunity for medical respite programs to review and implement aspects of the End-of-Life Care in Medical Respite Programs Guide to increase access to comprehensive end-of-life care for homeless adults, while learning from subject-matter experts and other programs on recommended practices for end-of-life care for homeless adults. As a result of the training series, programs will be aware of, and feel ready to, provide end-of-life care that considers the needs of their clients, community, and staff.
Whether your program is just getting started or well-established, this community of practice will offer important information and peer learning opportunities to support your work.

The deadline for application: Wednesday, December 31, 2025.

Schedule
This Community of Practice will take place from 2:00pm – 3:30pm ET on the following Mondays:
- January 12, 2026
- January 26, 2026
- February 9, 2026
- February 23, 2026
Time Commitment
One or more representatives from each participating organization will attend four 90-minute virtual sessions.
